This is Melissa Yuan, retina fellow at Stanford, in conjunction with G. Atma Vemulakonda, MD, a retina specialist at the Palo Alto Medical Foundation and Stanford. This video demonstrates a surgical posterior capsulectomy done at the time of a vitrectomy with membrane peel in a pseudophakic patient with a Vivity intraocular lens (Alcon).
After prepping and draping, we begin by inserting the 3 25-gauge trocars and cannulas in a beveled fashion. The infusion canal is inserted inferotemporally. All tracers are placed 3.5 mm from the limbus.
Anterior vitrectomy is performed. Then, the vitrector is used to cut a circular opening in the posterior capsule of about 4 to 5 mm in diameter. This is achieved by putting the cutter opening directly against the posterior capsule. This surgical capsulectomy improves the intraoperative view during peeling and prevents postoperative posterior capsular opacification.
