A 55-year-old male with poorly controlled diabetes, severe NPDR, macular edema in both eyes, and poor vision (20/800 OD, 20/400 OS) began treatment OU. After 6 treatments (A), the persistence of cystoid macular edema was concerning. Given this concern along with lackluster vision improvement and the appearance of intact outer retinal bands on OCT, the latter suggesting the possibility that better vision could be recovered with further resolution of fluid, the decision was made to switch to a different class of therapy in both eyes. One month following the first treatment, improvement of macular edema was seen OU (B). Further improvement in the retinal anatomy occurred subsequent to both the second (C) and third (D) treatments in both eyes. However, vision 1 month after the third treatment was 4/400 OD and 20/200 OS.
